## Purpose

Create a risk register that drives review decisions instead of hiding uncertainty.

## Preconditions

Apply `GOV-PROFILE-SKILL`; obtain the target, authority, and evidence needed for the Risk Register Skill procedure.

## Procedure

1. Identify risks from concrete assets, threats, dependencies, uncertainties, and planned changes.
2. Score likelihood and impact using declared scales; name evidence, owner, trigger, treatment, due point, and residual risk.
3. Link high risks to release gates and review the register when evidence or scope changes.
