1. Assume the change has failed materially and describe concrete user, data, security, operational, and delivery consequences.
2. Work backwards to plausible initiating events across inputs, dependencies, concurrency, permissions, configuration, and human operation.
3. Rank failure scenarios by impact, likelihood evidence, detectability, and recovery difficulty without inventing numeric precision.
4. Identify leading signals and the logs, metrics, traces, user reports, or integrity checks that would expose each scenario.
5. Define prevention, containment, rollback, and recovery actions with named owners or escalation paths.
6. Convert high-priority scenarios into tests, release gates, monitors, or explicit accepted risks.
7. Reassess the proposed design against a smaller, reversible, staged, or feature-flagged alternative.

## Decision gates

1. If a high-impact failure has no detection or recovery path, block release until one exists or risk is explicitly accepted.
2. If rollback could corrupt data or violate compatibility, require a forward-recovery plan and rehearsal evidence.
3. Proceed only when critical scenarios have an owner, observable signal, and tested or reviewed response.

2. Rollback is assumed but not executable: downgrade readiness and require a rehearsal or forward-recovery plan.
3. A critical scenario depends on an unowned external system: identify the escalation contract and block unsupported guarantees.
